Documentation for Appeals
When you need to submit medical record documentation with your appeal, include all pertinent documentation. The documentation should include the medical record detailing the service and the medical necessity of the service in question. In addition, providers should supply a separate statement indicating the specific information to evaluate in the medical record that supports the rationale for the appeal. This will help to ensure accurate, timely processing of your appeal.
On Friday, July 1 through Monday, July 4, 2016 the Common Working File (CWF) will experience dark days as it undergoes maintenance in support of the July 2016 release. During this time, our IVR will have limited functionality. The IVR will resume normal functionality on Tuesday, July 5, 2016, for your eligibility, claim status, or deductible inquiries. Throughout this period our internet portal, C-SNAP, will be available for all functionality except the additional claim status information providers can access related to duplicate denials.
